• Crisis intervention and 24-hour crisis support
• In-patient treatment programs
• Partial Hospitalization Programs (PHP)
• Intensive Outpatient Programs (IOP - coming Fall 2026)

3. Eligibility

You must be at least 18 years of age to use our services, except as otherwise permitted by law for minors with parental consent. By using our services, you represent and warrant that you have the legal capacity to enter into these terms and that you are not prohibited by law from receiving mental health services.

4. User Responsibilities

As a user of our services, you agree to:

• Provide accurate and complete information during intake and throughout treatment
• Disclose all relevant medical history and current medications
• Attend scheduled appointments or provide timely notice of cancellation
• Follow treatment recommendations and clinical guidance
• Maintain confidentiality of other patients and staff members
• Comply with facility rules and policies
• Treat staff and other patients with respect and dignity
• Not engage in illegal activities or substance use on our premises

5. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW:

• Bett-R.Life & Associates is not liable for any indirect, incidental, special, or consequential damages arising from your use of our services
• We are not responsible for any outcomes or results from treatment, as mental health treatment involves inherent uncertainties
• Our liability is limited to the amount paid for services received
• We are not liable for delays or failures in service delivery due to circumstances beyond our reasonable control

6. Confidentiality and Privacy

Your privacy is protected under HIPAA and applicable state laws. All information shared during treatment is confidential and will not be disclosed without your written consent, except as required by law. 7. Intellectual Property

• Co-pays and deductibles are the patient's responsibility
• Unpaid balances may be referred to a collection agency
• We reserve the right to suspend services for non-payment

10. Cancellation and Termination of Services

Either party may terminate the therapeutic relationship with written notice. We may terminate services if:

• You are non-compliant with treatment recommendations
• You engage in threatening or abusive behavior
• You violate facility policies or rules
• Your treatment goals have been achieved
• We determine that we cannot adequately meet your clinical needs

Upon termination, we will provide referrals to other providers and ensure continuity of care when clinically appropriate.

11. Disclaimer of Warranties

OUR SERVICES ARE PROVIDED "AS IS" WITHOUT WARRANTY OF ANY KIND, EXPRESS OR IMPLIED. WE DO NOT WARRANT THAT OUR SERVICES WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE. WE DISCLAIM ALL IMPLIED WARRANTIES INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.

12. Indemnification
